Aldermanic Acknowledgement Letter Form - Building Permit
If you have fallen victim to a scam, the City’s Business Affairs & Consumer Protection (BACP) is responsible for prosecuting businesses engaged in unlawful practices. In certain instances, BACP may also facilitate the recovery of restitution for affected consumers.
To initiate the process, a complaint can be filed either online or by calling 311. Following this, the consumer will receive a complaint affidavit by mail, which requires a detailed account of the incident.
The affidavit must also include a notarized signature to enable BACP to commence their investigation. It is crucial that this completed affidavit is returned within the specified timeframe to ensure the investigation can proceed without delay.

Please ensure you include your email address at the bottom of the form, as your permit will be sent to you via email. Please note that only block parties that have been active for a minimum of one year qualify for street closure approval.
